Certainly each jubilarian class is proud of at least one of its batch mates. Somebody whose accomplishments, especially in spite of life's adversities, serve as an inspiration to others.

Let him/her be recognized by the alumni, and serve as a beacon to us all! Nominate him/her to the "JSHS-QC Team 2014 Jubilarian Inspiration Awards".

The nominee should belong to one of the jubilarian classes this year (from classes 1969, 1974, 1979, 1984, 1989, 1994, 1999, 2004, and 2009). The nominee can be nominated posthumously.

The nominator can submit more than one nominations, but cannot nominate himself.

The nominator must submit the nomination form duly and accurately accomplished, with the current curriculum vitae of the nominee. In addition, the nominator may also submit documents supporting the nominee's accomplishments.

Hard-copy submission should be hand-delivered to any of this year's Execomm members. Soft-copy submission should be sent to JSHSQC.Team2014Awards@gmail.com.

Deadline of nomination is September 30, 2014.
